Whether your session is in-person or virtual, the sitter should be physically nearby and within earshot, ready to assist if needed. Their presence creates a supportive environment without stepping into the role of a therapist.

Some of the key roles a sitter plays during your session include:

- Being in direct contact with your clinician or therapist, should questions or concerns arise


The person you choose should be someone you trust to be nearby but not directly involved in the therapeutic process.

For many clients, choosing a sitter is straightforward—it could be a partner, a family member, or a close friend. However, if you don’t have a direct support network, finding the right person may take some additional thought.

- Trustworthy: Whether it's someone close to you or a vetted professional, the sitter must be reliable.

- Willing to focus on your physical needs: During the session, their attention should be on ensuring you’re physically safe and comfortable.

- Open-minded: It helps if they’re comfortable with medicine-assisted therapy or, at the very least, non-judgmental.

- Caring and patient: The ideal sitter will offer calm, steady support as you navigate your journey.

You’ll need your sitter to be present for about 1.5 to 2 hours during the session, so choosing someone who lives nearby or can easily make the time commitment is ideal.

Does the Sitter Have to Be the Same Person Every Time?

It’s beneficial to have consistency, but it’s not a strict requirement. If needed, you can have different sitters across your sessions. However, having someone familiar with the process can bring continuity and comfort to your experience.

If you feel comfortable, asking a close friend or family member can be a great option. Here’s an example of how you might approach the conversation:

“I’ve been working on personal growth for my mental health and am using ketamine therapy as part of my healing journey. I’d love if you could be there for me during my session—it’ll only take about 1.5-2 hours, and I trust you to support me in this process.”

It’s common for friends or family to have questions about what the role entails. We encourage you to share our sitter guide with them to help them understand how they can support you.

If your close network isn’t available, or if you’d feel more comfortable with a neutral party, you can consider hiring a professional sitter. Rates typically range from $18 to $35 per hour, depending on your location and the individual’s experience.

Let’s start with something easy. Can you tell us a little about your professional background and areas of focus?

I am a board-certified psychiatrist specializing in Integrative Psychiatry, Ketamine-Assisted Therapy, and Psychedelic Harm Reduction and Integration. I earned my MD from the University of Colorado School of Medicine with additional training in their Urban Underserved Track. After completing my psychiatric residency at Mount Sinai Hospital in New York City, I focused on underserved populations in East Harlem before turning to psychedelic-assisted therapy. I completed a fellowship in Integrative Psychiatry and received training in Ketamine-Assisted Therapy through the Integrative Psychiatry Institute and other renowned experts.

My clinical practice is heavily influenced by my long-standing interest in consciousness, psychedelics, and integrative medicine. I am also a graduate of Vajrayana Buddhist seminary and have over 20 years of intensive training as a meditation instructor. 

I'm best known for my work with ketamine-assisted therapy and served as the national Medical Director for Field Trip Health and at Nue.Life before joining the Innerwell team as their Chief Medical Officer.

How would you describe your approach to therapy?

My approach to therapy is collaborative, honoring client autonomy while fostering a supportive environment for healing, growth, and flourishing. I believe that each person's journey is unique, and therapy should be tailored to individual needs. I combine evidence-based practices with innovative approaches, focusing on the therapeutic alliance and holistic well-being.

How did you become interested in working with psychedelics including ketamine?

My interest in working with psychedelics, including ketamine, grew from my long-standing fascination with consciousness and integrative medicine. Early in my career, I worked with underserved communities, and I saw firsthand the limitations of traditional treatments for mental health conditions. I was drawn to the transformative potential of psychedelics, particularly their ability to offer healing in ways that go beyond conventional approaches. Through extensive training and personal exploration of meditative practices, I saw how these therapies could deeply enhance mental health care and foster lasting change.

What has been your most meaningful experience in working with psychedelics? 

This one is hard to answer, as there are so many! The general theme of these revolve around the profound power of these treatments to elicit deep meaning and lasting change for people who demonstrate a readiness for such.

What excites you most about the future of psychedelics?

Following the research, we're seeing an increase in potential indications for psychedelics. We're also seeing many new molecules and routes of administration. Furthermore, we're seeing a lot of creativity around the kinds or corresponding talk therapies utilized with these medications. To put it shortly, this is really the beginning of a major paradigm shift in how psychiatric care is delivered.

Ketamine has a longstanding track record of safety, having been administered to millions of adults and children since 1970. Notably, it differs from opioids and benzodiazepines in that it does not pose respiratory issues, even in higher doses.

Over the past three decades, extensive studies have investigated ketamine's efficacy in treating depression, anxiety, and PTSD. The findings consistently affirm its safety and effectiveness in addressing these conditions.

Your journey with ketamine begins at a low dose, allowing you to acclimate to its effects. At this level, ketamine induces a deep sense of relaxation, akin to enjoying a glass of wine. As your sessions progress, doses can be adjusted to align with your unique circumstances, condition, and treatment goals. Higher doses often manifest as dream-like experiences, where intricate imagery and altered self-perception may arise.

While most patients experience no side effects, those who do typically encounter brief and minimal issues lasting 1 to 2 hours. Common side effects include nausea, dizziness, headache, grogginess, and blurry vision.

Trust in the Process: You've been prescribed a legitimate, safe, and effective medicine tailored to your mental health needs. What to know if you’re nervous about your first Ketamine experience

At Innerwell, your safety is our top priority. We want to reassure all our patients that we take the utmost care in ensuring your safety. We have an extensive number of safeguards in place with ketamine treatment at Innerwell to ensure your well-being throughout your journey with us. 

Ketamine is a safe medication for patients who have undergone a thorough medical and psychiatric evaluation and do not have any contraindications to treatment. It is a remarkably effective medication for off-label treatment of several psychiatric conditions. In clinical practice, ketamine dependence and abuse are rare. Ketamine can be used safely for at-home treatment when the following guidelines are followed; 

All patients are screened for medical contraindications prior to initiating ketamine treatment. At the discretion of your prescriber, we may require specialist clearance from cardiologists, neurologists, and other physicians for patients who have any conditions that we determine can increase the risk of adverse events from ketamine (i.e., any history of significant heart, liver, lung, and kidney disease). Certain patients are not suitable for at-home ketamine treatment due to their medical or psychiatric history.

We require that patients stop or hold sedating and/or stimulating medications on the day of their treatment sessions to reduce the risk of adverse interactions with ketamine.

Ketamine can transiently increase blood pressure, usually in the range that can occur during exercise, and we therefore require patients to check their blood pressure before every treatment session to ensure that it’s in the lower range. Patients with a history of high blood pressure are required to be adherent to their antihypertensive medication regimen, and in certain cases, provide clearance from their PCP or cardiologist. 

We only prescribe oral tablets of ketamine at doses significantly below the amount used in general anesthesia and the level found in Matthew Perry’s bloodstream. All patients are carefully assessed throughout their program whether increases in their dose are clinically indicated. 

We require all patients to have a sitter (support person) physically onsite with them for every treatment session. As a reminder, our Sitter Guide can be found here. 

Patients should only take ketamine in a safe and secure setting at home, typically while lying on the couch or in bed. Patients should never take ketamine in or near a body of water such as a pool, hot tub, lake, or the ocean. Patients should also never take ketamine outside their home, or while driving or operating heavy machinery. Patients and/or their sitters should not hesitate to call 911 if they ever have any safety concerns during a ketamine treatment session.

Eye Movement Desensitization and Reprocessing (EMDR) is a unique and powerful therapy designed to alleviate the distress associated with traumatic memories. Developed by Francine Shapiro in 1987, EMDR has since gained widespread recognition and is now endorsed by numerous institutions, including the American Psychological Association. This article delves into the mechanisms of EMDR, its stages, and its scientific backing.

Eye Movement Desensitization and Reprocessing (EMDR) is a therapy developed to treat individuals who have experienced trauma. Unlike traditional talk therapy, EMDR does not primarily involve discussion of the traumatic events. Instead, it uses a structured approach where the therapist guides the client's eye movements while the client recalls distressing memories. The goal is to facilitate the adaptive processing of these memories, reducing their emotional impact and promoting psychological healing.

EMDR is recognized by the American Psychological Association and the World Health Organization as an effective treatment for trauma and post-traumatic stress disorder (PTSD).

Understanding how EMDR works involves delving into the realms of neuroscience and cognitive psychology. EMDR leverages the brain's natural healing processes, which is thought to be similar to the mechanisms involved in rapid eye movement (REM) sleep. According to the Journal of EMDR Practice and Research, the therapy aids in reprocessing traumatic memories to a less disturbing state by facilitating changes in the way these memories are stored in the brain's network.

EMDR therapy unfolds through eight well-defined phases, each critical to the therapeutic process. According to research published in the Journal of EMDR Practice and Research, these phases collectively aim to desensitize and reprocess traumatic memories.

The first phase involves gathering a comprehensive history and identifying target memories for treatment. The therapist assesses the client's readiness and formulates a treatment plan tailored to the client's needs.

During this phase, the therapist explains the EMDR process and techniques to the client, preparing them for the upcoming sessions. This phase is crucial for building trust and ensuring that the client understands what to expect.

The therapist identifies specific aspects of each target memory, including the visual image, negative beliefs, and physical sensations associated with the memory. The client rates the intensity of these aspects on a subjective units of distress scale (SUDS).

This phase involves the core of EMDR therapy. The therapist guides the client through sets of bilateral stimulation (typically, guided eye movements, but sometimes tapping or auditory tones), while the client focuses on the distressing memory. The goal is to reduce the emotional charge associated with the memory.

During the installation phase, the therapist aims to strengthen the client’s positive cognitive belief using bilateral stimulation. For instance, if the initial negative belief was "I am powerless", the positive belief might be "I am strong and capable."

The therapist asks the client to focus on sensations in the body while thinking about the positive belief and the target memory. The goal is to identify and process any residual physical tension or discomfort.

Each EMDR session includes a closure phase to ensure that the client returns to a state of equilibrium before ending the session. Techniques for self-soothing and stress reduction may be taught and practiced during this phase.

In the reevaluation phase, at the start of new sessions, the therapist assesses the progress made in previous sessions and plans future sessions accordingly.

EMDR’s efficacy lies in its unique mechanism of action, which blends aspects of various therapeutic modalities, such as cognitive-behavioral therapy (CBT) and psychodynamic therapy. The exact mechanism is still under scientific investigation, but several theories have been proposed:

Adaptive Information Processing Model: Proposed by Shapiro, this model suggests that EMDR helps reprocess traumatic memories by assimilating them into the brain's adaptive information network. Research supports this theory, highlighting changes in how memories are stored post-EMDR.

Working Memory Hypothesis: EMDR engages working memory by requiring clients to recall distressing memories while simultaneously performing eye movements. This dual-task engagement is thought to reduce the vividness and emotional intensity of the traumatic memories (Journal of Anxiety Disorders).

Bilateral Stimulation and Neuroplasticity: Bilateral stimulation, such as eye movements, is believed to induce neuroplasticity, facilitating the brain's capacity to form new neural connections and rewire itself in a more adaptive manner (Nature Reviews Neuroscience).

EMDR has shown efficacy in treating not only PTSD but also other conditions like anxiety, depression, and phobias. A meta-analysis published in JAMA Psychiatry found EMDR to be highly effective for PTSD. The therapy is also supported by the U.K. National Institute for Health and Care Excellence (NICE).

The ways we self-sabotage can be quite sneaky and clever.  


The process of integration becomes particularly vivid and rich when working with psychedelic experiences, because psychedelics can bring to light our fractured and exiled parts, at what can feel like speed-of-light time. Bring your journal with you and jot things down as they present throughout the day

Begin with 20 minutes per day. Set a time and write anything that comes to mind- it is important to write not just logistical parts of the day, but how you feel about the things you are writing, both then and now. 

Work with a therapist or counselor experienced in ketamine integration to receive guidance and support in navigating the integration process.

If we were excellent at solving our own problems, we would have done that by now. 

Asking for help and allowing yourself to be helped is hard. We can do hard things.

While integration can be a transformative process, it is not without its challenges. Common obstacles such as fear, doubt, or difficulty applying insights may arise. Ketamine therapy can offer a bird’s eye perspective of ourselves, our relationships, and our thoughts, and sometimes, what we see can be challenging to accept. Perhaps we have been controlling and hostile to our family members because we feel a deep sense of unworthiness, or perhaps we have been inattentive parents because we are burnt out and running on empty. Treat each insight that comes through as a piece of gold. We can only transmute pain into peace once we have honored and accepted the source of the wound. We can move towards freedom once we have a better understanding of the cages we’ve built in our minds. Challenges are a natural part of the integration process. Strategies for overcoming these challenges include seeking support from professionals or support groups, practicing self-compassion, engaging in self-reflection, and being patient with yourself throughout the journey.

I originally entered ketamine treatment with the main goal of learning- as a therapist-in-training, specifically working with psychedelics, I wanted to understand how the medicine felt so I could support future clients by experiencing what they experience. When I began treatment, I was newly single and had taken a leave of absence from school due to overwhelming feelings of hopelessness, so this seemed like a good time to research healing. 


Over the course of the first few treatments, I quickly came to understand that throughout my entire life, I had been feeling a deep sense of inadequacy and mistrust in myself, stemming from harsh, emotionally unavailable parents and other various traumas collected in the warzone that is adolescence on planet earth. I was unconsciously seeking a romantic partner to “save me,” and projecting onto them some made up fairytale which they would never live up to in real life. I also was allowing harmful and toxic relational dynamics in my life. Because I so desperately wanted to be cared for, I excused terrible behavior and abandoned myself in the process. I was trapped in a cycle of seeking reconciliation for my inner twelve year old, who had to survive alone and just wanted protection.
